A 67-year-old female with a history of approximately 5 prior strokes, hypertension, diabetes, and hyperlipidemia, was brought in today due to concern about ongoing speech difficulties since Wednesday. The patient has known left deficits from prior strokes. She was brought in by EMS and per her husband, she has been off for several days and decided to come to the hospital because it was not improving. She can answer questions but her husband is not at the bedside so it is difficult to determine a definitive baseline at this time. The patient notes she is not having pain anywhere and is not having other weakness or difficulty other than difficulty with her speech which is new aphasia, denies any recent illness, nausea vomiting fever chills, and no abdominal pain or other symptoms at this time. What are three nursing diagnoses for this patient? Please provide short-term goals and long-term goals for each diagnosis.
