If I'm correct it should be "Box 147, Rapid City, South Dakota."

The reason that is - is because we need a pause between when the city is being said and the area is being said.

You would write it like this

Box 147, Rapic City, South Dakota

The best way to help yourself remember where to put commas, say it outloud. You should be able to hear where you should space out, because I hope you don't talk in running sentences haha. Good luck
