This is not ethical. If the client is having any health or emotional issues, not answering the call bell is negligent.

Ignoring a patient when the patient needs attention is totally negligent and not inclusive in the ethical code of caregiving.  

A patient might be emotionally sick or physically sick and must be attended to every time he/she needs help or attention regardless of the number of times the patient requests or needs them.

Therefore we can conclude that ignoring the bell by the caregiver is negligent and Unethical.
