" I smiled I m doing great! How are you?" This smile is breaking my face; I don"t know if I can hold the tears back. I'm going to collapse like a bridge in an earthquake.

How does the difference between the characters spoken word and internal thoughts affect the story?

Answer:

it shows that the character doesn't want others to see how they are really feeling, this could be an act of foreshadowing meaning that later on they do end up showing their true feelings

Answer: The character's body language and words show that they are projecting this idea to the other person that they are okay although they’re falling apart trying to put up this front to convince the other person that everything is fine. If we didn’t know the character's internal thoughts we’d think that the character is genuinely okay and they already put everything in the past, but since we know the character's internal thoughts we see past the frail mask they are frantically trying to keep in one piece. This effects the story by adding tension and leaving us with deep anticipation for what happens next.
