The nurse should place the client in the orthopneic position to help alleviate the distress of a client with heart failure and pulmonary edema.

Pulmonary edema

Congestive heart failure frequently results in pulmonary edema. Blood can back up into the veins that carry blood into the lungs when the heart is unable to pump blood effectively.

Fluid is forced into the lungs' air sacs (alveoli) as the pressure in these blood vessels rises. The usual oxygen flow across the lungs is decreased by this fluid. Together, these two elements contribute to breathlessness.

Possible causes of congestive heart failure that results in pulmonary edema include:

  • Heart illness or a heart attack that causes the heart muscle to become weak or stiff (cardiomyopathy)
  • heart valves that leak or are constricted (mitral or aortic valves)
  • sudden, extreme hypertension (hypertension)
