An 84-year-old widow with dementia who had been living with her daughter before hospitalization is being discharged with a referral to the visiting nurse. When the nurse visits, the client is in bed sleeping at 10:00 am. Her daughter states that she gives her mother sleeping pills to stop her wandering at night. How should the nurse respond?

The nurse should explore hiring a home health aide to stay with the client at night. She has to offer her services in order to reduce the need for sleeping pills that are one of the causes of descrese of confusion. The only thing the nurse must take care of is the client's safety at night; she should help the family to reduce the night confusion that the widow experiences in the night time.
